The first days after discharge are when most setbacks happen. Our caregivers bring your loved one home safely, keep medications on track, watch for warning signs, and get them to follow-up visits — clinician-led support across Houston to make recovery smoother and prevent a return to the hospital.

Coming home should be the best part of recovery — but for many families, it's the most fragile. At Ama Ultimate Home Health Services, post-surgery and post-hospital recovery care is built to carry your loved one safely through those first vulnerable days and weeks. Led by Dr. Amaette Okon, DNP, PMHNP-BC, FNP, our caregivers bring more than two decades of clinical experience to every recovery plan.

Discharge papers rarely capture how demanding recovery really is. In the days right after a hospital stay, the risks stack up quickly — and they're the same risks that send people back to the hospital:

Each recovery plan is built around the procedure, the surgeon's instructions, and your loved one's home. Our post-surgery support commonly includes:

Whether your family needs a few hours a day or continuous overnight coverage, we tailor the schedule — including 24-hour and live-in care for the most fragile early days.

Recovery care here isn't handed off and forgotten. Every plan is designed and supervised by Dr. Okon's clinical team, so the support in your home is consistent and accountable. If a caregiver notices a concerning symptom — a wound that looks wrong, confusion, swelling, a missed dose pattern — it's flagged and escalated, not overlooked. Pairing day-to-day care with medication management oversight keeps the whole recovery on track.

Coordinating with your surgeon

We work alongside your surgeon and care team, not around them. Frequently asked questions

How quickly can recovery care start after discharge?

Often the same day. Once we know a discharge date, we can have a caregiver ready to bring your loved one home safely, settle them in, and begin support right away. When timing is tight, call us and we'll move quickly to coordinate.

Do you provide skilled nursing or wound care?

Our caregivers provide non-clinical support and reminders — mobility help, medication reminders, wound-care reminders, symptom watching, and meals — and coordinate with your clinical providers. Dr. Okon's team oversees the plan so anything that needs a nurse or physician is flagged and escalated.

Can recovery care help prevent another hospital stay?

Yes. Most readmissions trace back to falls, missed medications, infections caught too late, or missed follow-up visits. Consistent in-home support that watches for warning signs, keeps medications on schedule, and gets clients to follow-up appointments meaningfully lowers that risk.
